My Health Checks App:

This free smartphone app has been designed by Kent Community Health Foundation Trust to support patients following their NHS Health Check. The app allows patients to:

Keep a personal digital record of their NHS Health Check results and heart age

Set goals for themselves, like losing weight, getting more active or lowering their blood pressure

Get sensible advice, based on their results, to support them to make the change to a healthier lifestyle.

The app is free to download on iPhone, iPad and all android phones. To download, patients should search the app store or Google Play for ‘My Health Check’.

How do I get an NHS Health Check

If you're in the 40-74 age group without a pre-existing condition such as stroke, heart disease, diabetes or kidney disease, you can expect to receive a letter from the practice inviting you for a free NHS Health Check every five years.

SAMs hopes that you understand that there will have to be a staggering of invite letters due to the numbers involved but SAMs are increasing its nursing capacity to continue to deliver this service.
